Among different infertility treatment methods there are two main groups: methods directed at restoration of natural fertility of a couple and methods using the technology of assisted reproduction.
Infertility Treatment. Restoration of natural fertility.
- Attempt to restore passability of uterine tubes by non-surgical way
- Surgical restoration of uterine tubes passability
- Correction of endocrine disorders
- Restoration of damaged spermatogenesis
Infertility Treatment. Assisted Reproduction Technologies
- Intra-uterine insemination (IUI) by spouse’s sperm
- Intra-uterine insemination by donor’s sperm
- In-Vitro fertilization and embryo transfer (IVF-ET) in different variants
Until recently the treatment of uterine tubes infertility was limited by endless repeating of physiotherapeutic procedures, both time-consuming for doctors as well as for patients, hydrotubation cycles combined with antiinflammation therapy. Efficiency of this treatment concerning restoration of uterine tubes passability is considerably low, while consequences in form of additional strains of tubes sections and loss of their functioning ability are quite serious, since it all makes all other attempts of surgical treatment senseless.
The latter did not fulfill expectations of leading specialists in infertility treatment. So, it was proved, that impassability of uterine tubes, caused by “external” factors (joints), the possibility of pregnancy after restoration-plastic operations is quite high and makes up 20 – 70% (depending on the qualifications of the surgeon).
But in case, if impassability of uterine tubes is caused by adhesive processes in tubes sections, the possibility of pregnancy even after microsurgical operations is only 0 – 5%. At the same time operations treating the infertility by womb cutting are considered to be traumatic and are connected with definite risks for the patient. Therefore, recently there is a tendency to limit cavity operations and increase the number of small “laparoscopic” operations, i.e. operations conducted during surgical laparoscopy.
Treatment of “unclear” (idiopathic) infertility
Treatment of “unclear” (idiopathic) infertility represents a serious problem as well. Sometimes effective is treatment with hormones which is directed at ovulation stimulation, although the patient has normal ovulation. But in the majority of cases methods of artificial fertilization are used. Most effective among them is extracorporeal fertilization.
In general, when we look how infertility treatment is conducted nowadays, it is important to stress, that instead of routine methods with low efficiency new methods, based on latest achievements of medicine and technology, with high efficiency, are gaining popularity. In these conditions the most suitable strategy concerning infertile couples will be more active approach which uses all possibilities of modern hormonotherapy, methods of artificial fertilization, mainly extracorporeal fertilization in different variations. In order to treat infertility successfully, its causes should be diagnosed correctly. Rational treatment way should be implemented especially on the stage when treatment method is chosen.
